If your child’s pediatrician is part of our Clinically Integrated Network, then our Care Management Team will be working hand-in-hand with their office to effectively manage patient care.
What is the role of Care Management?
Care Management is an important component in helping participating physicians manage their patients more effectively. For patients working with our Care Management Team, All Children’s Care Network will proactively work with participating providers to identify and connect them with appropriate Network resources to coordinate and further enhance care. This includes assisting patients in obtaining needed services to improve quality while addressing gaps in care.

After hours? How to know where to go for care?
Call your Pediatrician
- You are worried and aren’t sure what to do next
- Cough, congestion, runny nose, sore throat
- Fever (even >103!)
- Vomiting/diarrhea, possible dehydration
- Simple injuries, cuts, splinters, and sprains
- Minor animal bites, minor burns
- Ear pain, ear infection, or sinus infection
- Headache, body aches, belly pain
- Allergies and/or rash/hives
- Stitch/Staple Removal
Consider Urgent Care when the office is CLOSED and you are worried

1st – Call your pediatrician to speak with the ON-CALL service

2nd – Consider Urgent Care Center
Go to the ER
- Difficulty breathing (persistent)
- Seizures
- Uncontrolled Bleeding
- Ingested a poison, drug, or unknown substance
- Serious injuries (visibly broken bones, unconsciousness)
- Major burns
*Call 911 for life-threatening events
