Who Can Participate?
The Care Network welcomes pediatric practices committed to delivering high-quality, coordinated care for children and their families. Participation is open to pediatric practices and organizations focused on collaboration, quality improvement, and community-based care.
Participation Requirements
One delegate from a participating practice must:
- View and disseminate quarterly education
- Validate practice information quarterly
- Attend bi-annual data-review meetings
- Engage with Care Network on improvement opportunities
24/7 Virtual Pediatric Support for Eligible Patients
For qualifying patients of participating practices, Blueberry Pediatrics offers 24/7 virtual access to board-certified pediatricians, helping extend care beyond the office and support coordinated, value-based care. This service strives to reduce unnecessary trips to the ER.
Value-Based Contracting
Value-based contracting shifts many of the traditional fee-for-service incentives in healthcare. Primary care, preventive services, and early intervention help prevent unnecessary and costly acute care in the future. That premise underlies this care network’s approach to value-based contracts.
